Nothing to print or send us. The public Notice to Consumer and LDA contract are references—not forms to complete here. You do not need to print, complete, sign, or return either sample. Your initial SuperFast Divorce® or Complete Divorce™ application is a request for review, not a completed service contract. If the requested work fits and you choose to proceed, we provide your separate required notice, contract and service terms through the enrollment process.
Saving a copy is optional. Client, service, fee, signature, and signing-date fields are blank. Your completed agreement—not this sample—will state your selected services and charges.

Notice or contract—what’s the difference?
The Notice to Consumer comes before the contract or payment. It identifies the provider and explains nonattorney limits. You read it, answer based on the first conversation, sign and date it, and receive a copy before another contract is offered for signature. This is a separate step in your client process—not something completed by reading this page. California notice requirements.
The LDA Contract for Self-Help Services sets out the work, fees, cancellation rights, and required notices. California requires an LDA who contracts to provide self-help services to use this prescribed written contract. Additional terms cannot replace it or reduce mandatory protections. Your application and complete service terms
The initial online application for SuperFast Divorce® or Complete Divorce™, for divorce or legal separation, is a request for review. Your separate completed LDA contract identifies your selected service, charges, and payment arrangement. You do not need to fill out or send us the public sample first.
Our Master Services Agreement and applicable attachments explain additional responsibilities, cancellation and privacy policies, electronic communications, arbitration, and limitations of liability. You can review and keep the complete applicable terms before any signature or payment. The public reference PDF does not include our full MSA or your client-specific details. An attorney can advise you about their legal effect.
The initial application does not collect payment or produce a signed LDA contract. Required notices, service agreements and payment instructions are provided separately in the required sequence before paid work begins. You receive copies at the required steps, including the fully executed LDA contract before services begin. Nothing needs to be printed or returned from this public reference page.

Before choosing a document preparation service
Look beyond a polished website or a low price. Ask clear questions about the people, the paperwork, and the service you are buying:
Who is responsible for preparing my documents?
Ask for the name and role of the person doing the document work—not just the company or its founder. Ask about relevant experience and how the work is checked. If that person performs services requiring LDA registration, ask for their individual registration number, county, and expiration date. A company registration does not replace an individual registration where the law requires one.Where can I verify the LDA registration and bond?
Contact the clerk in the county of registration. Have the person's full name, business name (if any), registration number, county, expiration date, business address, and phone number ready. Ask the county how to confirm the current registration record and applicable bond coverage. Use the Los Angeles County contact information or the California Secretary of State's county-officials directory to find another county's clerk. Verification may require a call or written request; not every county offers an online LDA search.What do clients say about the service?
Read reviews across more than one platform, such as Google and Yelp. Look for clear communication, follow-through, and help with the document process—not just a star rating. If you are hiring an LDA for self-help services, ask to see the separate Notice to Consumer and prescribed LDA contract. If they are missing, ask why before signing or paying. A job title is not the same as a registration.

Common questions
Can I cancel after signing?
You may cancel for any reason within 24 hours after you and the LDA have both signed the contract. Give written notice as described in the contract. Part III explains the full cancellation rules, refund rights, and limited conditions governing charges for work already performed. BPC §6410.
Who signs, and when can work begin?
You sign as the applicant client, and Fast Divorce signs the LDA contract. You receive the fully executed copy before services begin. A spouse who voluntarily engages us receives their own required documents before work requiring their participation, directions, or signature. A nonparticipating spouse does not automatically become our client.
Does a payment plan or upgrade replace the state contract?
No. Your completed contract and applicable additional terms identify your service, charges, and payment arrangement. Confidential Divorce™ is an optional upgrade for suitable Complete Divorce™ matters, with additional terms when applicable. It is not available with SuperFast Divorce® and does not guarantee sealed records or confidentiality.
Do I need to print, sign, or return the public PDF?
No. It is an unsigned reference, not your completed agreement. Your initial SuperFast Divorce® or Complete Divorce™ application is for review. If the work fits and you choose to proceed, your actual contract is completed through the separate enrollment process. Reviewing or downloading the sample does not hire Fast Divorce. You may save it for your records, but nothing needs to be printed or sent to us from this page.
